Officially launched in July 2025, the initiative is designed to transform hypertension management by offering continuous, technology-enabled support for patients struggling with uncontrolled blood pressure a condition that impacts more than 30% of adults in Ohio and contributes to thousands of preventable hospitalizations each year.
UH operates one of the largest healthcare networks in Northern Ohio, combining advanced clinical innovation with research and academic excellence. As the RPM program scales across the UH Primary Care Institute, it will extend to nearly 100 practice sites and empower more than 400 providers to proactively manage patients with chronic conditions.

By integrating HealthSnap’s platform directly into UH’s existing workflows and electronic health record (EHR) systems, clinicians can deliver proactive, data-driven care without disruption to their daily practice.
