Clarify Health, a tech-enabled healthcare outcomes enterprise specializing in referral intelligence, quality tracking, and provider network solutions, has finalized its acquisition of Loyal Health Holdings, Inc. (“Loyal”), a dedicated digital patient activation platform. The transaction establishes the healthcare sector’s first closed-loop network intelligence engine a unified software platform built to determine optimal patient care pathways, seamlessly guide individuals to the appropriate providers, and systematically evaluate clinical and operational outcomes.

The contemporary healthcare landscape remains challenged by fragmented patient navigation tools and data silos, which frequently lead to out-of-network leakage and incomplete care transitions. Loyal’s Care Activation Platform addresses these pain points by supervising more than 80,000 provider and facility profiles while facilitating millions of patient search queries and automated scheduling actions annually for major health systems nationwide. By integrating these consumer-facing front-end capabilities with Clarify’s proprietary Meridian® intelligence ecosystem, the combined entity provides healthcare delivery systems with comprehensive, end-to-end management over patient access points, referral trends, and overall market expansion.

Commercial clients using solutions from either organization will continue to receive uninterrupted service, product availability, and technical support. Financial specifics regarding the corporate acquisition were not disclosed. Legal counsel for the transaction was managed by Orrick, Herrington & Sutcliffe LLP on behalf of Clarify, and Goodwin Procter LLP on behalf of Loyal, with OM Partners serving as Loyal’s primary financial advisor.
