Neuroglee Health, a leader in value-based dementia care, announced an expanded collaboration with Mayo Clinic to deliver comprehensive cognitive and dementia care to patients, along with dedicated support for their care partners. This builds upon the organizations’ collaboration that began with developing Neuroglee Connect™ solution, which is guided by principles from the HABIT Healthy Action to Benefit Independence & Thinking® program to provide patients and their families access to industry-leading treatments to promote the highest level of cognitive function and independence in people with Mild Cognitive Impairment.

Patients with cognitive impairment and dementia can be enrolled in Neuroglee’s program for longitudinal virtual cognitive and dementia care delivered by a specialized team of advanced practice nurses, licensed social workers, pharmacists, and care navigators, with supervision from neurologists and geriatric psychiatrists, all delivering high-touch, wraparound support 24/7.

Neuroglee’s whole-person-focused solutions emphasize care partner enablement, in-home care continuity, and hospital utilization avoidance. The rollout aligns with Neuroglee’s selection for the Center for Medicare & Medicaid Innovation’s GUIDE (Guiding an Improved Dementia Experience) Model, a first-of-its-kind payer initiative to bring comprehensive value-based dementia care to Medicare beneficiaries. Neuroglee will be enrolling GUIDE-eligible patients across Mayo Clinic locations and other partner sites at no cost to the patient or family.

The average annual per-member cost to Medicare for patients with dementia is $22,000, almost three times the cost of members who do not have Alzheimer’s or another dementia. However, with Neuroglee’s comprehensive, proactive dementia care model, payers and risk-bearing entities have the ability to save over $6,300 per member per year through early detection and comprehensive longitudinal care.
