GroundGame.Health (GGH), an industry leader at removing barriers to care, has engaged Socially Determined to deliver individual and community-level social risk data for a 155,000-member population.

The proprietary risk data and scoring platform SocialScape® will help GroundGame.Health focus its efforts through identification and prioritization on high-risk members for social care intervention.

GroundGame.Health’s enhanced visibility into social risk supports closing care gaps and improving outcomes for the country’s most vulnerable populations.
